All the ones I've used have been good with foreigners who don't speak Japanese.

For your first time, it'd probably be easier for you to go with one of the ones with an English website with an English reservation form - they'll reply to you in English. 

As for the boys, I've just been using Google Translate to converse with them and it's been fine.

I really liked the Dojo-group in Kyoto. A few of the guys I hired are also available in Osaka. Don’t even bother with the form on the website. Just email them to set it up. Almost everyone in Japan speaks limited English. Some will say they don’t speak any, but in my experience, after I spoke a little Japanese to them, they suddenly could also speak some English. And there’s always Google translate.

In Tokyo, I used Re:Born and Gym-K. I had amazing sex with guys from both agencies. I wasn’t happy with Tokyo Kids. If you like skinny twinks, you might enjoy it more than me.

Have fun. I enjoyed myself so much, I’m planning on going back for two months.

A coupe of questions for people about the boys at the agencies

1.  When a boy is listed as "straight", what are they usually willing to do?   Just seems a little strange to me that  aStr8 guy would be willing to kiss and suck.   

2.  Of the boys that list Uke, what percent really do it.    If I select a boy that has just Uke listed and he won't do it, I would be disappointed in the arrangement.   

My trip is 3 months out, but I have been looking at the sites for my favorites.  (thou I realize in 3 months boys will com and go).   But think my best get is to pick one that does both Uke and Tachi.

Thanks for the useful tips guys.  I think I’ll try to make 3-4 appointments during my 5 days in Osaka/Tokyo.  Maybe 2 with twinks and 1 with this hot pornstar Masato from lang.dgdgdg.com  .  Tempted to do an overnight with him.  I’ve always wanted to fool around with a hot straight guy just for a night.  My trip is about 15 days away.  Is it too early to make arrangements by email or should i lock everything down asap?  Thanks guys, you’re a wealth of info!

1. Yup, kiss and suck. They are gay4pay and just there to earn money but like most Japanese, they'll give it a good go because they're predisposed to providing good service  but I find they aren't really good at doing either so I only go for a straight guy if he bottoms too, personally.  

2. If ◎ or 〇, almost always. If △, depends on how big you are. I think all the sites have a disclaimer that the boy has a right to refuse if their butt isn't in good shape that day or if you're too big, but I've only been notified once (out of ~20-30 visits) and even then he did it.... Oh wait I just remembered there was one other time and that guy didn't do it. I was disappointed, yup, but we did other stuff. I believe both were △.

Oh I like picking out my faves in advance too, lol. Certainly adds to the anticipation of the trip.

Usually the guy's profile has his availability and I think they only release the following week's a few days prior but no harm in trying and they'll tell you if it's too early.

When doing some research i found out dgdgdg in Osaka update their schedules for the next week at 6pm on Fridays, not sure if it's the same for all locations
