Prison Break - Final Show?

I saw the final season show of Prison Break and enjoyed it. The show is one of my favorites. The last few episodes I have not been able to keep up with in any way. It just changed things so fast I my mind felt like it was on a roller coaster. Even with that, I enjoyed the season. But, what of the last scene? Does this mean he is leaving the show? Was this the series final episode?
