Basically some form of liquid such as silicone, oil, paraffin, etc is injected into the outer skin of the penis which then form a bulge or lump (granuloma) thus enlarging the penis and most often disfiguring it! It can be undone with surgery wherein the skin of the penis is removed (degloved) and some form of skin transplant is grafted onto the penis. Typically a flap of skin from the scrotum is used for the skin transplant. The result varies but the patient does lose his foreskin and remains circumcised. A eunuch has his testicles removed, ( castration) not his penis. Too graphic?? You can always google for more info.
